SEMFC Application Instructions
- Select the Application Form (.pdf) from the SEMFC Forms/Documents page, and Adobe Acrobat will be launched.
- To fill the form in online, place your cursor over the various
fields and type the corresponding information. Date would be the
date you are filling out the application. All dates must follow the
format of MM/DD/YYYY. (You may also print the blank application and
fill it in. In this case, just select print.)
- Ensure you fill in ALL applicable information.
- After completing the form, select print.
- Mail the completed and signed form, along with copies of the following items to the address below:
- Medical Certificate (if applicable)
- Pilot Certificate (if applicable)
- BFR (if applicable)
- Driver's License
- Social Security Card or a valid second form of identification
- Signed Article 16 from the Bylaws
- Signed and completed Annual Insurance Questionnaire found under Forms
- Completed KRST Badge Application - PDF, which
can be found here:

- Deposit in the form of a check, written to SEMFC, for the
amount of membership fee plus equity buy-in (see Fying Regulations and
Operating Rules for the specific amount)--your check will NOT be cashed
until you are accepted into Club membership

- You may also give the completed package to one of the Board members.
- If your package is incomplete, it will be mailed back to you.
- If your package is complete, it will be reviewed at the
next Board meeting, and the Secretary will get back to you with your
application status within a week of the Board meeting.
